Constant out-of-memory errors

Hello

I am receiving a near constant stream of PHP 'out of memory' errors, as well as occasional 500 errors, on both my website and the control panel.

Here is an example error message:

Fatal error: Out of memory (allocated 21495808) (tried to allocate 122880 bytes) in /home/training/public_html/modules/user/user.module on line 1778

I have checked phpinfo(), and it has the memory limit set to 40MB. The out of memory errors occur at much lower memory levels, typically 8 to 12 MB.

Please advise.
