While I have owend two .xxx domains for a short while, I did not until just now, know that you had to do this application form thingy.

Any email message they would have sent me, I would have ignored.

How can I get this email message 'resent' so I can get that application form done this time?

The membership code is given by the Central Registry for XXX domains - ICM.  You'd have to contact them directly to get one:
